H1N1 Vaccination Information

In order to receive H1N1 vaccination, individuals must be pre-registered through the Ohio Department of Health. If you or any of your family members fall into an identified high-risk group then you should go to the following website to be pre-registered:

https://h1n1vaccine.odh.ohio.gov/

Those identified as being at the highest risk are the following groups:
- Pregnant women
- People who live with or care for children younger than 6 months of age
- Health care and emergency services personnel with direct patient contact
- Children 6 months through 4 years of age
- Children 5 through 18 years of age who have a chronic medical condition

Even those who do not fall into high risk groups are eligible for vaccination and should pre-register if they wish to receive vaccination. Ohio Family Practice Centers hopes to be receiving supply of H1N1 vaccine at some point in the future and we will notify you through this website and at the office when/if this occurs. As listed previously the CDC has an excellent website with extensive information regarding H1N1 (commonly known as Swine Flu).
