Yearly physicals for adults as well as well-baby and well-child visits

Our goal is to maintain and promote your good health through regular preventative medicine visits and routine screening tests as indicated by age as well as family and personal medical history. You will be assigned to one of our health care providers and we will do our best to maintain continuity so that you can develop a close trusting relationship with your provider. In the event that you need same-day acute care we will do our best to schedule you with your regular care provider.

General Information – Because of the volume of patients we have, we ask you arrive ten minutes before your scheduled appointment. We do not overbook or double-book appointments, and we try as best we can to stay on schedule. However, medical emergencies take priority and there are times patients must be taken out of order.

We ask that you give us a 24 hour notice for cancellations, as this allows us to give that appointment to those who are waiting.

Payment and Insurance Information – Payment is expected at the time of visit. We accept most major forms of insurance and credit cards. If you cannot pay, please let us know, as we can arrange for a payment plan.

Please have your most current insurance card with you. We are required to verify your insurance and to collect a co-payment, if required.
