Through the patient portal you can access your medical records, request refills, request appointments and even send a message to your medical team. Available to you 24 hours a day.
We're a Certified Patient-Centered Medical Home:
Ohio Family Practice is a recognized, Patient Centered Medical Home (PCMH) practice. PCMH is a model of care that puts patients at the forefront of care and works to build better relationships between people and their clinical care teams.
Research shows PCMH practices improve the patient experience and staff satisfaction, while reducing health care costs. Our PCMH recognition means we have made a commitment to providing a patient-centered approach to care that results in our patients being happier— and healthier.
We're an Evidence Based Practice:
An Evidence Based Practice aims to provide the most effective care that is available, while improving patient outcomes. As health professionals, part of providing a professional service is ensuring that our practice is informed by the best available evidence. As an Evidence Based Practice we respect our patients’ unique situation, preferences and values. It is not a static model; rather, Evidence Based Practices continually incorporates updated research.
"Our goal is to maintain and promote your good health through regular preventative medicine visits and routine screening tests as indicated by age as well as family and personal medical history." - Dr. Kelli Sabin
